I hereby authorize and give my consent to Motor Ace Philippines, Inc. and MAPI Lending Investors, Inc. to collect, store, transmit, use and distribute disclose, share, retain, dispose, destroy and process my Personal Information and/or Sensitive Information contained in my Buyer Statement/Customer Record Form or any other forms or document that I have or will submit to Motor Ace Philippines, Inc. and MAPI Lending Investors, Inc. whether manually or electronically, pursuant to my lending/financial needs, in the procurement of motorcycle, for any of the following purposes prescribed by the DATA PRIVACY ACT 2012 and its Implementation of Rules and Regulations:

· To process application and other information related requests;
· To administer all my lending related documentations but not limited to Chattel Mortgage and Promissory Note;
· To receive servicing and support for my lending needs;
· To be included in marketing and promotional advisories; and
· To be a respondent in Motor Ace Philippines, Inc. and MAPI Lending Investors, Inc. research, profiling and data analytics to improve my lending/financing experience.

I understand that any information provided to Motor Ace Philippines, Inc. and MAPI Lending Investors, Inc. is protected. Motor Ace Philippines, Inc. and MAPI Lending Investors, Inc. will only collect my Personal Information and Sensitive Personal Information through secure means, i.e. in person at Motor Ace Philippines, Inc. and MAPI Lending Investors, Inc. branches, via recorded line through Customer Care Hotline and In-house Collection, e- channels Payment, MAPI Lending Online Platform and website.

I understand that this authorization/consent shall continue to be in effect throughout the duration of my lending and/or until expiration of the records retention limit set by the relevant lending laws and regulations for account closure, and the period set until destruction and/or disposal of my records, unless earlier withdrawn in writing.
